If you have a work item for deleting a node in your inbox, you have to reject it twice before it is actually rejected.
To reproduce
- copy a node, then activate it
- delete the node, then activate the deletion
- open the inbox: a work item for the activation is in the box - reject the item
- open the inbox again
-> the work item is still there (see screenshot) - reject the item again
-> now it's finally been rejected
