Chopped-off upload button in media upload field

      The "upload another" button and the "select file to upload" buttons in the media/file upload field are currently chopped off.

      While you're at it: please also rename the button to "Upload different" instead of "upload another", since the latter suggest that you can add an additional image (I'm aware I've requested something different in the past).
