 Description   

The 2FA login form presents a third text field for the user to enter their one-time password.

Unlike the existing username and password fields, this field doe not have an icon.
As a placeholder, this field contains the request "Please input your security code here" while the other two simply have labels, "Username" and "Password".

To present a consistent experience to all users, I think a suitable icon should be added and a shorter placeholder label used.
