Add a folder (test) to the DMS, add a document to it, then activate the document without first activating the folder.
This will generate an error:
Can't activate: : info.magnolia.cms.exchange.ExchangeException: 2 errors detected: Message received from subscriber: Activation failed | /test on magnoliaPublic8081 Message received from subscriber: Activation failed | /test on magnoliaPublic8080
(this is a setup with 2 public nodes)
now this is already a bug in my opinion, it should either not let you activate or (better) activate the necessary folders automatically.
Worse however, the folder is now corrupt: you can't activate it including subnodes. What does work I found out is activating the folder seperately, then activate its content separately.
Deactivating didn't solve the issue.
