The problem comes from the task trigger on update to 4.5 in MailModuleVersionHandler:

.addTask(new BootstrapSingleModuleResource("Templates configuration","Reconfigure the templates configuration.","config.modules.mail.config.templatesConfiguration.xml"))

The custom configuration added in templatesConfiguration is overridden. We must bootstrap each default template configurations separately.
