1) Button label missing
The label is missing, so button is just a black square. It is not clear what it is.
Previously there was "Add" label since 6.2. In M5 we have had just + sign. See the screenshots for more info.

definition here is not decorated and field label is no longer hardcoded.
Still exists in 6.2.8 bundle.
