This control should be called "selection" not check/radio
It should also include a drop down list of options
The field name should be named "Key" (see previous comments) and the decription should be alötered like for the input box.
The first entry should be the "selector title" which should be named "Label" to be consistent with the rest of the controls
What is Legend - it doesn't do anything
Display orientation should be named "Orientation" and should have to options horizontal and vertical, not a checkbox
Labels and Values is good, but it should state somewhere that you need to enter one line per Label
Enter default value for field if any: should say "The option with the default value will be selected when the form is displayed"

Please capitalize only the first word of each label

Regarding multiline entries . If you have many entries this is not maintainable. I would suggest to use a single text area to enter both, with the label first, a separator and a value second; if no value is present, use the label as value, e.g.:

pizza: 1
chicken: 2
ice-cream: 3

I don't think we need ":" as part of the label, so it should work like I suggested.
